Level three: Intensive – My Plan+

The child's needs are more complex and require an assessment to understand the range, depth or significance of the needs which may require a more intensive response.

At Level three, a My Plan+ (including a My Assessment) should be used to help all those involved (or who may need to be involved) to understand the child or young person’s aspirations and needs and how they impact on daily life.

A My Plan+ should be reviewed at regular intervals to map progress and make changes where needed.
