Early Identification and Intervention Tool

If your child has a diagnosed Learning Disability and/or (suspected) Autism but also shows signs of challenging behaviour, the professionals involved with your child (Social Worker, CAMHS LD Nurse, Early Years Service etc.) may discuss with you about adding them to the Early Identification and Intervention Tool.

This is a monitoring tool which allows professionals across the system in health, education and social care to know who your child is and should their needs begin to increase. This will then allow them to work together in a timely and effective way to put the best support in place for you and your child.
