Protect a vulnerable adult

If a vulnerable adult has been hurt or is at risk, and abuse or neglect is not the cause, contact the Adult Social Care Helpdesk. You can find out more about how to know if an adult is at risk on our website.

If you are being abused, or you think someone else is, call the police on 101 or in an emergency dial 999.

There are different processes you should follow depending on the needs of the individual and what you require.

Safeguarding

If you are a professional and suspect someone is experiencing or at risk of abuse, please complete a safeguarding referral form via the Adult Social Care portal.

Individual with eligible support needs

If you think that the person you’re working with may be eligible for support from adult social care, you can submit a referral. This will help us decide what the next steps should be.

You will need to provide us with the following information:

  • name
  • address
  • date of birth
  • contact details
  • GP details.

We'll also need details of:

  • any relevant health conditions
  • current living and support arrangements
  • what they wish to achieve
  • confirmation that you’ve sought their consent and if not, why not.

Individuals with difficulty managing day-to-day activities

A therapy referral can be made online. This is managed by the Integrated Community Teams who bring together community nurses, occupational therapists, physiotherapists, social workers and reablement workers to work as one team to serve a local area.

They can support people to live more independently at home if they are finding it hard to manage day-to-day activities due to a physical, mental or sensory impairment, illness or old age.

Individual in an unpaid caring role

If an individual looks after someone with support needs, you can refer them for a carer's assessment. This will look at what support they need in their role as a carer and how we can help.
