What will my assessment consider?

During your Adult Social Care Assessment, we will look at things including:

• your home and how you are coping in it
• your health and emotional wellbeing
• how you can best be supported to live as independently as possible
• what care and support options are available to you.

We can work with you to create a support plan and personal budget designed to meet your needs and help you to stay as independent as possible.

Support may be freely available in the community, or we may need to look at support which has to be paid for. Working together ensures you have control over any decisions.
