Referral process
You should contact either the adult's or children’s helpdesk for advice and guidance, and a home adaptations referral.
Basic requests may go through a Fast-track process and be dealt with over the phone by the Helpdesk. Otherwise, referrals will be passed to an Occupational Therapist(OT), who will consider the following situations:
- those needing short-term support to recover from an illness or injury
- those needing support due to a long-term health condition or disability
- those who have a terminal diagnosis and need support at home for the remainder of their life.
The OT will complete an assessment in your home to determine your needs and consider any requests you might have. They may recommend specialist equipment, a home adaptation, or refer you to another service such as physiotherapy or your GP.
