How to complete and submit the Mileage Reimbursement Claim Form

Parents / Carers
  1. Please look at the example of a completed form published on the GCC web page. Example of a completed claim form
  2. Download and print a copy of the Claim Form for the relevant month, or complete electronically. If you require a paper copy, telephone 01452 425334 and a member of the ITU payment team will send a copy by post.
  3. Complete and sign the form for the month.
  4. Hand, or email the completed form to your School or College to verify and sign.
  5. The School or College must email the verified form to Gloucestershire County Council who will then check the submitted form against attendance data. Any queries will be addressed to the claimant as soon as possible.
School / College
  1. Please check the days claimed against the school / college attendance data.  This must be done accurately to avoid fraudulent claims. 
  2. Please sign the box to verify that the check has been completed, this can be done electronically or by hand. 
  3. Please scan paper forms or forward electronic versions and send to: ituinvoices@gloucestershire.gov.uk
  4. Please complete this process promptly to avoid delay in parents/carers receiving their payments, this can cause hardship or affect ability for the families transporting the child to school / college.
